For trauma survivors, hospital discharge is rarely the end of the story. It's the beginning of a long, often overwhelming recovery journey—one marked by medical follow-up, mental health needs, housing or job insecurity, food insecurity, and the logistics of rebuilding a life. And yet, that journey is often unsupported.

Today, we're proud to share the launch of TandemGuides—a first-of-its-kind program that expands recovery care beyond the hospital walls.

They’re juggling appointments, medications, insurance paperwork, and basic needs like food, childcare, and transportation—often without help.

TandemGuides brings a new layer of dedicated, real-world support designed to walk beside survivors after discharge. That includes:

  • Fast-track access to trauma-informed therapy and behavioral health support

  • Navigation for housing, food assistance, childcare, employment, and more

  • Help with insurance benefits, scheduling, and referral coordination

Every patient journey is different—so TandemGuides meets each individual where they are, with support tailored to their goals, barriers, and healing.

Grounded in lived experience & built to scale

At the heart of TandemStride’s platform is peer mentorship—a powerful model that connects patients with trained mentors who have survived traumatic injury themselves. These mentors offer understanding, hope, and hands-on support during one of the most vulnerable chapters in a survivor’s life.

TandemGuides builds on this foundation, adding clinical resource navigators, licensed social workers, and streamlined pathways to services that matter most. In early pilots, 40% of survivors were connected to support within 48 hours, and the program has already helped reduce avoidable hospital readmissions by up to 20%.

The result is a trauma recovery model that’s not just compassionate—it’s effective, scalable, and ready to meet the moment.
